Position Summary:
This is a career-building opportunity for a Physician Assistant (PA-C) seeking a better balance between professional impact and personal life. In this role, you will provide on-site rounding care, manage chronic conditions, and build meaningful relationships with patients and care teams. You will have the autonomy to manage your day, grow your clinical expertise, and make a measurable difference in senior living settings.
Responsibilities:
• Provide on-site rounding care in assisted living, memory care, and long-term care communities.
• Perform assessments, chronic disease management, and preventative care.
• Develop and adjust patient-centered care plans.
• Collaborate with physicians, nurses, and interdisciplinary teams.
• Communicate with families, caregivers, and facility staff.
• Reduce hospitalizations through proactive care delivery.
• Complete documentation, medication management, and lab coordination.
Qualifications:
• Active Minnesota Physician Assistant (PA-C) license in good standing.
• Experience in geriatrics, family medicine, or post-acute care preferred.
• Strong clinical and communication skills.
• Valid driver’s license and reliable transportation.
• Passion for relationship-based senior care.
