Compensation: $70,000-$102,000 annually

What You'll Be DoingDeliver Personalized Rehabilitation
- Implement treatment plans developed by licensed Physical Therapists
- Provide mobility training, strengthening exercises, gait training, and balance activities
- Educate patients and caregivers on safety and home exercise programs
- Monitor progress and communicate updates to supervising therapists
- Help patients maximize independence and functional abilities
Collaborate With An Outstanding Team
- Partner closely with Physical Therapists, nurses, and interdisciplinary clinicians
- Document visits accurately and efficiently
- Contribute to exceptional patient outcomes
- Support patients throughout their recovery journey
Required Qualifications
- Active Minnesota Physical Therapy Assistant license
- Reliable transportation, insurance, and valid driver's license
- Strong communication and interpersonal skills
- Passion for patient-centered care
Preferred Qualifications
- Home health experience
- Experience with medically complex or geriatric populations
- Desire to work independently while collaborating closely with a supportive team
