What You’ll Do:
- Implement care plans crafted by a licensed PT to improve strength, safety, and function
- Provide home-based therapy including mobility support, ROM, balance training, and gait work
- Track and report progress using streamlined EMR software
- Teach exercises, safety tips, and self-care strategies to patients and caregivers
- Work closely with PTs, nurses, and the broader care team to ensure top-tier outcomes
Qualifications:
- Current Minnesota Physical Therapy Assistant license
- Reliable transportation, insurance, and valid driver’s license
- A warm communicator with strong motivation to deliver patient-centered care
- Home health experience preferred, with thorough onboarding provided
