Job Title: Start of Care Nurse (RN) – Admissions Specialist

Key Responsibilities:
- Conduct comprehensive Start of Care (SOC) assessments and OASIS documentation
- Develop individualized care plans for new clients
- Educate patients and families on their care path and available resources
- Collaborate with interdisciplinary teams and providers
Required Qualifications:
- Active RN license in Minnesota
- Proven OASIS documentation and SOC experience
- Home health background preferred
- Reliable transportation and valid driver’s license
