Job Summary
We are seeking a dedicated and energetic Hospice Registered Nurse (RN) to join our compassionate team. In this vital role, you will deliver expert nursing care to hospice patients, ensuring comfort and dignity at the end of life. You will coordinate with families and interdisciplinary teams to develop personalized care plans, manage symptoms, and provide education on disease progression and comfort measures. Your expertise in acute and post-acute care settings will help us deliver exceptional hospice services that prioritize patient needs and uphold the highest standards of care.
Responsibilities
- Conduct comprehensive patient assessments, including vital signs, symptom management, and physiological evaluations to inform individualized care plans.
- Administer medications and treatments in accordance with physician orders while monitoring for adverse effects.
- Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and utilization management efforts.
- Provide emotional support and education to patients and their families about disease progression, symptom control, and comfort measures.
- Document all patient interactions accurately in medical records while adhering to HIPAA regulations.
- Manage inpatient and outpatient hospice visits, ensuring seamless continuity of care across settings.
- Participate in diagnostic evaluations and post-acute care planning to optimize patient outcomes.
Skills
- Hospice experience is preferred but not required.
- Skilled in patient assessment, vital signs monitoring, discharge planning, and case management.
- Familiarity with medical terminology, medical records management, intake procedures, utilization management, diagnostic evaluation, and post-acute care coordination.
- Experience in outpatient settings, including IL, AL and personal homes.

Pay: $70,000.00 - $80,000.00 per year
