LICENSED VOCATIONAL NURSE (LVN) - HOME HEALTH

ABOUT THE ROLE

The Licensed Vocational Nurse provides direct patient care under the supervision of a Registered Nurse. The LVN follows the established plan of care, performs ordered treatments within the LVN scope of practice, monitors and reports the patient's response, educates patients and caregivers, and maintains accurate and timely clinical documentation. This position is well suited for a dependable, compassionate nurse who values teamwork, clinical precision, and consistent follow-through.

RESPONSIBILITIES

Provide direct patient care within the California LVN scope of practice and under RN supervision.

Implement nursing care and treatments according to the current plan of care and physician orders.

Observe and assess the patient's response to treatment, medication, and changes in condition.

Report significant findings, emerging needs, and changes in condition promptly to the supervising RN and physician as directed.

Provide patient and caregiver education related to diagnosis, treatment, medications, safety, and self-care.

Administer, monitor, and document medications within the LVN scope of practice and agency policy.

Provide pain and symptom management interventions as ordered and evaluate the patient's response.

Complete accurate, timely, and complete clinical notes, flow sheets, incident reports, and other required records.

Document care consistently with the plan of care and agency documentation requirements.

Communicate and coordinate with the RN, physician, therapists, aides, pharmacists, vendors, and other members of the care team.

Use equipment and supplies safely, effectively, and efficiently.

Follow infection-control, standard-precaution, emergency-preparedness, and patient-safety procedures.

Participate in quality improvement activities, education, and competency validation.

Perform other clinically appropriate duties assigned by the supervising RN.

CLINICAL SKILLS

Depending on the assigned patient and validated competency, duties may include vital signs and focused assessments; respiratory and oxygen-related care; diabetes and glucose monitoring; medication administration by authorized routes; wound and skin care; catheter, ostomy, gastrostomy, and enteral-feeding care; specimen collection; mobility support and transfers; and patient and caregiver education.

QUALIFICATIONS

Graduate of an accredited vocational nursing program.

Current California LVN license in good standing.

At least one year of nursing experience.

Home health, community health, hospice, or medical-surgical experience preferred.

Demonstrated ability to follow a plan of care and work under RN supervision.

Strong verbal and written communication.

Good organization, time-management, and documentation skills.

Commitment to accepted professional standards and patient-centered care.

Valid driver's license, reliable insured vehicle, and ability to travel to patient homes.

Ability to use clinical equipment safely and perform the physical duties required for field nursing.
