What are the responsibilities and job description for the Speech-Language Pathologist (SLP / Speech Therapist) -- PRN Home Health position at Home First Home Health Care?
Home First Home Health Care is seeking a PRN Speech-Language Pathologist to provide individualized care to patients in their homes.
Join our interdisciplinary team and help patients improve communication, cognitive-communication skills, and swallowing function while supporting greater safety and independence at home.
What you’ll do:
- Evaluate patients’ speech, language, cognitive-communication, and swallowing needs.
- Develop and carry out individualized treatment plans.
- Provide treatment consistent with physician orders and the patient’s plan of care.
- Educate patients and caregivers on strategies, exercises, and precautions.
- Monitor progress and update treatment recommendations as appropriate.
- Communicate with physicians and other members of the care team.
- Complete accurate evaluations, visit notes, reassessments, and discharge documentation within required timeframes.
What we’re looking for:
- Current Florida Speech-Language Pathologist license.
- Current CPR certification.
- Strong assessment, communication, and organizational skills.
- Ability to work independently while collaborating with an interdisciplinary team.
- Dependability and a commitment to patient care and timely documentation.
- Ability to travel to patient homes within your assigned service area.
- Comfort using electronic documentation systems.
- Home health experience preferred; experience with Axxess is a plus.
