On-Site Medical Assistant (MA) & Clinic Coordinator
We are seeking an innovative, tech-forward Certified Medical Assistant (CMA/RMA) to serve as the critical operational anchor for our primary care and digital health clinic in Lake Forest.
The ideal candidate will not only deliver elite clinical support to our Nurse Practitioner (NPs) and Physicians but will also serve as the main trusted interface for our senior living residents. This role requires high-touch empathetic communication, complete technical agility, and strict adherence to healthcare compliance.
Job Qualifications
Required Experience & Education:
- Certification: Valid Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential.
- Experience: Minimum of 2–3 years of clinical experience, with a heavy preference for individuals who have operated inside Life Plan Communities, CCRCs, Assisted Living, or Geriatric Primary Care settings.
- Technical Agility: Proficiency with Epic, PointClickCare (PCC), Practice Fusion, and MS Office. Proven capability to troubleshoot basic cellular or software issues on-site.
- Communication: Exceptional interpersonal skills and emotional intelligence tailored for building deep, respectful relationships with an affluent, highly engaged senior demographic.
Key Responsibilities
1. High-Touch Clinical Support & On-Site Operations
- Manage daily patient flow, rooming, and point-of-care diagnostics (including vital sign mapping, vaccinations, injections, and point-of-care testing) inside the shared, non-exclusive Lake Forest Place clinical suite.
- Coordinate clinic hours and equipment setups efficiently around external shared-space providers (such as visiting dental, podiatry, and audiology groups).
- Work hand-in-hand with on-campus wellness nurse and existing Medical Director to ensure unified clinical transitions.
2. Multi-EMR Data Traffic Management & Continuity
- Maintain error-free documentation for all Independent Living clinic encounters within our group’s primary EMR.
- Sync diagnostic clinical profiles within Epic to support continuity of care.
- Log, document, and cross-reference clinical encounters for Assisted Living, Memory Support, and Skilled Nursing residents directly within PointClickCare.
3. Remote Patient Monitoring (RPM) & Triage Navigation
- Serve as the primary on-site fulfillment and education lead for our cellular Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) cohorts.
- Issue pre-configured, "plug-and-play" cellular medical devices directly to seniors, delivering clear, baseline instruction to maximize month-to-month compliance.
- Monitor the centralized software triage dashboard, highlighting critical vital parameter alerts for NP/Physician review and tracking cumulative care minutes.
4. Compliant Resident Engagement & Concierge Logistics
- Act as the friendly, accessible face of the clinic to introduce our services to residents and answer inquiries from the Resident Advisory Committee.
- Build trust with all stakeholders and relationship-driven care with patients
- Manage the enrollment logistics and after-hours scheduling pipelines for our elective, private-pay concierge urgent care membership model.
