What are the responsibilities and job description for the LPN Clinical Intake position, on Weekends. Phoenix, AZ position at Home Caregivers Partnership LLC?
Graduate of an approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing.
Minimum of one (1) year experience in nursing, preferred.
Ability to exercise initiative and independent judgment.
You will be responsible for initiating and processing patient referrals to assure that the patient needs are met. Establishes and maintains ongoing relationships with referral sources, payors and case managers.
Coordinating patient care.
Assuring the development, implementation, and updates of the individualized plan of care, which would entail communication with all physicians involved in the plan of care and integration of orders from all physicians involved in the plan of care including those orders related to medications.
Maintains accurate and complete records of observations, treatments and care of patient.
Work remotely and cover all three offices Glendale, Phoenix and Gilbert.
Work weekends from 9:00am to 5:00pm .
Home Health and Hospice experience is preferred.
