J&M Homecare Services is looking for a kind-hearted, hardworking, and self-starting professional to join our growing team as a Client Management Specialist. This role is ideal for someone who thrives on helping others, takes initiative, and enjoys building meaningful relationships in the community.
You’ll support both our Homecare and Strategic Partnership programs by conducting in-home client assessments and Extended Care Management (ECM) duties throughout Marin and Sonoma Counties. As part of California’s CalAIM initiative, you’ll play a key role in connecting individuals with the care, resources, and support they need to live healthier, more independent lives.
What You’ll Do
- Conduct in-home assessments and develop individualized care plans
- Enroll and support members in the CalAIM ECM program
- Coordinate medical and social services for clients
- Coordinate in-home care services
- Build and maintain relationships with clients, families, and healthcare partners
- Maintain accurate documentation and compliance with program guidelines
What We’re Looking For
- 2 years of Care Management experience (preferred)
- CHW (Community Health Worker) training/certification a plus
- Compassionate, reliable, and proactive personality
- Strong communication and organizational skills
- Proficiency in Microsoft Office 365
- Reliable transportation (local travel required 25–50%)
- Bilingual preferred
