Position Overview
The Clinical Team Lead works directly with our Regional Clinical Director, to support clinical operations, maintain compliance, and elevate team performance.
This is a hands-on leadership role—ideal for an experienced home health RN who is comfortable managing documentation quality, supporting field staff, and ensuring the office is always survey-ready.
Key Responsibilities
- Support daily clinical operations across the San Diego office
- Review and ensure accuracy of OASIS, plans of care, and documentation
- Maintain compliance with Medicare, state, and ACHC standards
- Provide real-time clinical guidance to field nurses and therapists
- Participate in case conferences and interdisciplinary team meetings
- Support onboarding, training, and performance of clinical staff
- Drive timely documentation (24-hour expectation) and accountability
- Identify and resolve clinical and documentation issues proactively
- Assist with survey readiness, audits, and quality improvement initiatives

- Documentation is timely, accurate, and audit-ready
- Strong coordination between intake, clinicians, and leadership
- Clinicians are supported while maintaining accountability
- Office remains consistently survey-ready
- Patient outcomes and satisfaction continue to improve
Qualifications
- Active Registered Nurse (RN) license in California
- Home Health experience required
- Strong knowledge of OASIS, Medicare Conditions of Participation, and compliance
- Prior leadership or team coordination experience preferred
- Experience with EMR systems (WellSky preferred)
- Strong communication, organizational, and problem-solving skills
