Job Summary
Home Care Now is seeking a compassionate and clinically strong Home Health RN/LPN to provide one-on-one care in a home. This role is ideal for a nurse who values autonomy, patient relationships, and the ability to deliver skilled care in a home-based setting rather than a facility environment.
What You’ll Do
- Travel to patient homes to provide skilled nursing care
- Assess patient condition, vital signs, and response to treatment
- Administer medications
- Complete accurate, timely documentation for each visit
What We’re Looking For
- Active RN license in the state of employment
- Current CPR or BLS certification
- Level 2 Background
- Strong assessment and communication skills
- Ability to work independently in a home-based care setting
- At least 1year of Home health experience
- Experienced with trach g/j-tube
Schedule and Pay
- Both days and Nights available 7am to 7pm/7pm to 7am
- Flexible scheduling options may be available
- Pay range: $32 to $42per hour
