Home Care Network, Inc. is seeking a Licensed Practical Nurse (LPN) to provide Home Health Private Duty Nursing (PDN) services in and around Scioto County, OH for a pleasant, alert 17-year-old male with complex medical needs and loving family.
Critical Qualifications:
- Demonstratable experience to expertly provide one-on-one skilled nursing care for a pleasant, alert 17-year-old male with complex medical needs, including quadriplegic spastic cerebral palsy, seizure disorder, Crohn's disease/ulcerative colitis, and G-tube dependence.
- Compassionate, dependable LPN with strong assessment skills and experience (2 years highly desirable) or willingness to learn and perform:
- Medication administration and G-tube feedings, monitor vital signs, and assess for changes in condition while documenting skilled nursing care.
- Seizure precautions
- Respiratory treatments
- Airway management and clearance techniques, including frequent deep oral suctioning, cough assist treatments, percussion vest therapy, nebulizer treatments, and ongoing respiratory monitoring.
- Total care needs assistance, including transfers, bathing, dressing, oral hygiene, skin care, and incontinence care for a non-verbal, non-ambulatory client.
- Willingness and ability to accompany client to medical appointments as needed, providing skilled nursing care during transportation and specialty visits.
- Aptitude to work independently in the home while collaborating with the family and clinical team to ensure safe, high-quality patient care.
- 1 or more years paid experience in home health agency or equivalent is highly desirable, but we are willing to train the right LPN if needed.

Qualifications
- Active Ohio LPN (or RN) license
- Minimum of 1 year paid experience in home health agency / visiting nurse setting or direct care of patients with complex medical needs.
- Strong medication administration skills
- Reliable transportation and punctuality
- Active CPR and First Aid certificates highly desired (for speed to start care), but agency will provide renewal/recertification if required.
- Good interpersonal communication skills - written and verbal.
- Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care documentation), accountable, friendly and accommodating, respectful.
- Ability and willingness to successfully perform all required duties including essential physical demands (listed below).
- Current driver's license or state ID, plus reliable transportation, are required.

Starting Pay Rate:
- LPNs: $28 - $32 per hour
- RNs: $35 - $40 per hour

Position Summary
A successful PDN at Home Care Network, Inc., performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and agency. All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN/RN delivers care to clients as delegated by the RN Case Manager.
Reports to: RN Case Manager; Clinical Administrator/Director of Nursing
Essential Functions/Areas of Accountability:
- Provides professional nursing care under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per agency guidelines.
- Provides skilled nursing visits for clients as directed by the RN Case Manager. Follows the care plan established by the Registered Nurse.
- Recognizes and reports changes in client condition to the Case Manager and physician, as directed.
- Communicates information on a timely basis, consistent with agency policy and urgency of the situation. Communication is verbal, as needed, and through complete and accurate documentation in the clinical record.
- Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Case Manager.
- Individuals with the license designation of "LPN M-IV" may perform venipuncture for the purpose of obtaining blood samples for laboratory analysis.
- Promotes personal safety and a safe environment for clients and coworkers.
- Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures.
- Recognizes and responds appropriately to potentially unsafe situations.
