Responsibilities:

• Provide skilled nursing care to seniors in their homes, assessing their physical, emotional, and social needs
• Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary team members
• Administer medications, treatments, and therapies as prescribed by physicians, and monitor patient responses
• Educate patients and families on disease management, self-care, and wellness strategies
• Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services
• Maintain accurate and detailed records of patient interactions, assessments, and care plans
  
  
  

Requirements:

• Current RN license in the state of Wyoming
• Minimum of 0 years of experience in home care or a related setting (new graduates welcome to apply!)
• Ability to work flexible hours, including evenings, weekends, and holidays
• Valid driver's license and reliable transportation (mileage reimbursed)
