Certified Nursing Assistant (CNA) – Home Health Telehealth Assistant
Position Summary
The Certified Nursing Assistant (CNA) supports the home health therapy team by assisting with telehealth physical therapy visits in patients' homes. This role serves as the in-home support person for the remote therapist by helping facilitate virtual visits, obtaining and documenting vital signs, assisting patients with technology set-up, and providing camera guidance during home safety and mobility assessments. The CNA helps ensure efficient, high-quality patient care while promoting patient safety and comfort in the home setting.

Works under the direction and supervision of licensed therapists, registered nurses, and agency clinical leadership in accordance with Montana regulations, CNA scope of practice requirements, and agency policy.

Essential Duties & Responsibilities
- Travel to patient homes as scheduled to assist with telehealth therapy visits
- Set up and connect telehealth equipment (tablet, phone, laptop, hotspot, etc.) for virtual therapy appointments
- Assist patients in joining telehealth sessions and troubleshoot basic technology issues
- Obtain and document vital signs including:
- Blood pressure
- Pulse
- Respiratory rate
- Oxygen saturation
- Temperature
- Weight (if applicable)
- Assist therapist with camera positioning and guided visual assessments during visits
- Perform camera tours of the patient’s home environment as directed by the therapist to assess:
- Mobility pathways
- Fall risks
- Bathroom safety
- Transfer areas
- Durable medical equipment placement
- General home safety concerns
- Assist patients with positioning, transfers, and mobility activities within CNA scope of practice and agency policy
- Report observed patient concerns, changes in condition, or safety issues to supervising clinician promptly
- Maintain professional communication with therapists, nurses, patients, and caregivers
- Ensure patient privacy and HIPAA compliance during all telehealth interactions
- Accurately complete required documentation and visit notes per agency standards
- Maintain telehealth equipment and report malfunctioning devices or connectivity concerns
- Participate in team meetings and ongoing training as assigned
- Current Montana Certified Nursing Assistant (CNA) certification required
- Current CPR certification required
- Valid driver's license, reliable transportation, and current auto insurance required
- Ability to travel throughout assigned service area
- Basic understanding of telehealth technology and mobile devices
- Strong communication and interpersonal skills
- Ability to work independently in patient homes
- Home health, long-term care, or outpatient experience preferred
- Frequent standing, walking, bending, lifting, and transferring assistance up to 50 pounds
- Ability to navigate patient home environments safely
- Ability to assist with patient mobility, transfers, and positioning activities
- Primarily field-based in patient homes
- Frequent interaction with elderly and medically complex patients
- Exposure to varying home environments and conditions
