Overview
Health Care Connectors is seeking a compassionate and skilled Home Health Nurse Practitioner to join our growing clinical team. The ideal candidate is clinically strong, organized, and passionate about delivering high-quality, patient-centered care in the home setting.
Key Responsibilities
- Perform comprehensive patient assessments in the home, including physical exams, medication reviews, and chronic disease management.
- Act as the primary care provider for patients who do not currently have a PCP, ensuring continuity of care and ongoing clinical oversight.
- Complete and maintain OASIS documentation accurately and on time (required).
- Develop, implement, and evaluate individualized patient treatment plans.
- Collaborate with RNs, therapists, physicians, and other interdisciplinary team members to coordinate care and improve patient outcomes.
- Provide education to patients and families to support disease management and promote independence.
- Participate in in-office case conferences, training, and team meetings (approx. 25% of workload).
- Utilize Homecare Homebase (HCHB) for documentation and workflow (preferred).
- Ensure compliance with all federal, state, and agency standards and regulations.
- Provide on-call support if needed (optional—customize as needed).
Qualifications
- Active, unrestricted Nurse Practitioner license in Connecticut (CT).
- National certification (FNP, AGNP, or equivalent).
- Minimum 1–2 years of clinical experience; home health experience strongly preferred.
- OASIS experience required.
- Homecare Homebase (HCHB) experience preferred.
- Excellent clinical judgment, communication, and organizational skills.
- Valid driver’s license, reliable vehicle, and ability to travel throughout the Connecticut service area.
- Ability to work independently in a field-based environment.
Job Type: Full-time
Pay: $120,000.00 - $150,000.00 per year
