What are the responsibilities and job description for the CHW (Peer Support Specialist) position at Home At Last CDC?
Provide ongoing expertise and support through comprehensive assessment, planning, implementation and overall evaluation of individual client needs.
Essential Functions:
- Performs intake
- Complete an individualized care plan
- Mentor, motivate, and empower clients
- Assess and provide appropriate linkages and referrals to needed services
- Meet with participants weekly in person, virtually, or by phone
- Document all client interactions
- Search for community resources
- Communicate with either the Parole Officer or other Social Service Providers
- Accompaniment to appointments
- Transportation assistance and coaching.
- Maintains a detailed case file of every client assigned for service
Minimum Qualifications - Knowledge, Skills and Abilities Required
- A history of incarceration
- High School Diploma or equivalent
- Must complete live scan/background clearance
- Have demonstrated a period of stability and successful reintegration into the community following incarceration
- Work a flexible schedule (weekends and evenings)
- Good oral and written communication skills
- Reliable transportation is required
- Knowledge of HMIS
