What are the responsibilities and job description for the Case Manager position at Home At Last CDC?
Under the direction of the Program Manager, the case manager provides ongoing expertise and support through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of this position is to enhance the quality of client management and satisfaction, and to promote continuity of care and effectiveness through case management, utilization review and ongoing interaction with client. The case manager is responsible for monitoring the functional operation of the residential program and facility under the supervision of the Program Manager to ensure safety and regulatory compliance.
- Ensures the safety of participants, guests and property
- Control contraband in facility through general observation, facility searches, and client searches
- Coordinate the integration of social services and case management into the client’s care, placement and follow-up services.
- Conduct detailed review of client’s skills, areas of expertise as well as job history.
- Assess client needs throughout participation in any program operated by Home At Last.
- Serves as client advocate for outside services and treatment where applicable.
- Refers cases where client would benefit from outside counseling to ensure self-sufficiency.
- Communicates with either Parole Officer or other Social Service Providers regarding the treatment, advancement or clinical needs of client.
- Investigates and reports adverse occurrences, and works with other staff to identify resources, clinical plans and treatment that benefits client.
- Assess clients care throughout the continuum of service to align with Mental Health regulations.
- Mobilizes resources needed for healthcare, job training, job placement and permanent housing.
- Maintains a professional relationship with client and outside providers to ensure a client’s successful deployment into mainstream society.
- Maintains a detailed case file of every client assigned for service
- Ensure facility cleanliness by assigning and monitoring client work, morning duty, and extra duty assignments
- Performs intake
- Make periodic tours of the facility and the grounds to ensure high level of security by observation and corrective action, if necessary.
- Attend all staff meetings and training as directed
