Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patient health by checking vital signs, observing changes in condition, and reporting findings to healthcare professionals
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when required
- Administer medications accurately following care plans and HIPAA privacy regulations
- Maintain a clean and safe environment through regular cleaning, laundry, and organization of living spaces
- Support clients with cognitive challenges such as dementia or Alzheimer’s by implementing memory care strategies and engaging in meaningful activities
- Document care activities meticulously, update care plans regularly, and communicate effectively with family members and healthcare teams
Requirements
- Proven experience in home & community care, assisted living, nursing homes, or group homes preferred
- Demonstrated ability to work with individuals with disabilities, developmental delays, autism, or dementia
- Strong patient observation skills to identify changes in condition or behavior promptly
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
- Knowledge of medication administration protocols and vital signs monitoring
- Compassionate attitude with excellent communication skills and a commitment to providing respectful, person-centered care
