What are the responsibilities and job description for the Occupational Therapist - Home Health position at Holzer Health System?
Works under the supervision of the Director of Home Care. Performs direct patient care within the guidelines of the referring physicians to evaluate, design and reassess age-specific treatment plans to meet the assessed needs of pediatric to geriatric patients; and performs adequate documentation.
Supervises Occupational Therapy Assistants and unlicensed support. Assists in formulation of departmental and hospital-wide PI activities.
Education, Work Experience and Licensure:
• Bachelor’s/Master’s degree from an Occupational Therapy Program this is accredited by the American Occupational Therapy Association.
• Current Ohio and West Virginia Physical Therapy license in good standing is required.
• May be entry, but one year experience preferred.
• Current CPR certification is required.
• Valid driver’s license, proof of auto liability insurance and clean driving record. A clean driving record includes, but is not limited to the following: no OVI, no reckless operation, and no failure of physical control on driving record. If such violations should occur after employment, employee may not continue working in the Home Care department. Citations will be reviewed on a case by case basis. No criminal record as validated by criminal background check, including fingerprinting.
