Role Description
This is a full-time remote role for a RN Case Manager - Hospice. The RN Case Manager will be responsible for providing comprehensive nursing care to hospice patients, including developing and implementing care plans, coordinating with healthcare providers, and managing patient transitions. Daily tasks include discharge planning, utilization management, and ensuring that patients receive appropriate, continuous care.
Qualifications
- Nursing and Medicine skills
- Experience in
providing comprehensive nursing care to hospice patients
- Strong Case Management skills
- Excellent communication and organizational skills
- Ability to work independently and remotely
- Experience in the hospice or palliative care setting is a plus
- Valid RN license in Texas
