Role Description The Hospice Nurse is a full-time, on-site position located in Philadelphia, PA. This role provides direct nursing care to hospice patients in homes and facilities, including comprehensive assessments, symptom management, and administration of medications and treatments. The Hospice Nurse develops, implements, and evaluates individualized care plans in collaboration with physicians, interdisciplinary team members, patients, and families. Daily responsibilities include monitoring changes in patient condition, providing education and emotional support to patients and caregivers, and documenting all care in accordance with regulatory and organizational standards. The Hospice Nurse participates in interdisciplinary team meetings, coordinates services such as home care and equipment needs, and supports patients and families through end-of-life decision-making and bereavement transitions.

Qualifications
- Strong clinical foundation in Nursing and Medicine, with the ability to perform comprehensive assessments, manage complex symptoms, and respond promptly to changes in patient condition.
- Demonstrated expertise or strong interest in Palliative Care, including pain and symptom management, comfort-focused interventions, and holistic care at end of life.
- Experience in Home Care and Direct Patient Care, providing compassionate, person-centered support in patients’ homes or residential settings.
- Current, unrestricted Registered Nurse (RN) license in the state of Pennsylvania; hospice or palliative care certification (CHPN) preferred.
- Excellent communication, active listening, and teaching skills for supporting patients, families, and caregivers in emotionally challenging situations.
- Ability to work collaboratively in an interdisciplinary team, manage time effectively, and prioritize a dynamic caseload.
- Reliable transportation and willingness to travel locally within the Philadelphia, PA service area as required.
- Basic life support (BLS) certification; knowledge of hospice regulations, documentation standards, and EMR systems is an advantage.
