POSITION SUMMARY
The Registered Nurse Case Manager position is responsible for providing comprehensive case management services in alignment with the American Case Management Association (ACMA) Standards of Practice, New Mexico Board of Nursing (NM BON) regulations, Det Norske Veritas (DNV) accreditation requirements and Critical Access Hospital (CAH) standards as regulated by the Centers for Medicare & Medicaid Services (CMS). This position collaborates with an interdisciplinary team to ensure appropriate utilization of services, compliance with regulatory requirements, safe transitions of care and optimal patient outcomes. This position serves as a clinical advocate, compliance steward and systems navigator, addresses the unique challenges of rural healthcare to improve outcomes, reduce avoidable hospitalizations and connect patients with appropriate community resources. MINIMUM MANDATORY QUALIFICATIONS
EXPERIENCE:
- Minimum of two (2) years previous nursing experience.
- Associate’s Degree in Nursing from an accredited college or university.
MANDATORY KNOWLEDGE, SKILLS, ABILITIES AND OTHER QUALIFICATIONS:
- Current and valid nursing license in the State of New Mexico or compact state.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Proficiency in electronic health records (EHR) systems.
- Adherence to Holy Cross Medical Center (HCMC) policies surrounding patient safety, infection control and Health Insurance Portability and Accountability (HIPAA).
- Strong oral and written communication skills in dealing with a variety of individuals.
- Compassionate, patient-focused and adaptable to the dynamic needs of a rural healthcare setting.
- A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers.
- Basic Life Support (BLS)
