MAJOR RESPONSIBILITIES/ESSENTIAL FUNCTIONS
Position Summary:
Responsible for the evaluation of referred patients to assess their medical condition and functional limitations, restrictions and complaints. To establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance, under the administration of the Medical Director.
- Written evaluation of new physical therapy patients.
- Reassessment on ongoing patients.
- Develop and adjust patient treatment plan.
- Develop objective and measurable goals with specific time frames.
- Patient, family and community education.
- Provide rehabilitative physical therapy care to inpatients and outpatients inclusive of, but not limited to modality/exercise treatments, wound care, mobility and gait training.
- Documentation of progress notes and flow sheets/charts according to guidelines of the department, Medicare, Worker’s Compensation, or other specific payors.
- Written discharge assessment notes.
JOB QUALIFICATIONS
Minimum Education:
- Bachelor of Science degree from an APTA approved school of Physical Therapy.

Minimum Work Experience and Qualifications:
- Ability to communicate effectively verbally and in writing.
- Experienced preferred (will consider new grad).

Required Licensure, Certification, Registration or Designation:
- Valid California Physical Therapist License
- Eligible for membership in the APTA.
- Must adhere to code of ethics of the APTA.
- Basic Life Support (BLS) from the American Heart Association (AHA)
- Valid Los Angeles County Fire Card (required within 30 days of employment)
- Assault Response Competency (ARC) (required within 30 days of employment)
