What you will be doing:

• Plan, organize and direct home care services
• Complete initial assessment of patients to determine home care needs
• Regularly re-evaluate patient’s nursing needs
• Initiate a plan of care and make revisions as patient status changes
• Counsel the patient and family in meeting nursing and related needs
• Provide health care instructions to the patient, as appropriate per assessment and plan of care
• Communicate with a physician regarding the patient’s needs
• Obtain and receive physician’s orders as required

What we are looking for:

• 1 year of EMR systems is preferred
• 1 year of experience as a registered nurse in the state of California
• BLS certification required
