What Will You Do?
As the part-time Behavioral Health Specialist, you will work closely with the Social Services Manager for Care Resources. Duties include:
- Provide in-person and virtual counseling for participants
- Advocate on behalf of program participants
- Conduct comprehensive participant assessments, including screens for depression, anxiety, and cognition. Make recommendations based on these assessments.
- Communicate with facilities (AFC/AL/SNF) to provide behavioral health interventions for their staff to use with participants.
- Provide education and support for family members/caregivers as well as Care Resources staff
- Work 3 weekdays per week at the Care Resources facility in Kentwood, MI
- Master's degree in social work and fully licensed in the state of Michigan (LMSW) required
- Proven experience providing counseling services, behavioral health treatment plans, and behavioral interventions
- Minimum of 1 year experience working with a frail or elderly population is preferred
- Strong computer skills to effectively utilize virtual meetings as well as Microsoft Word and Excel
- Proven ability to work independently as well as within an interdisciplinary team setting
