Clinical Case Manager – Mental Health Services
Full-Time | Part-Time | Contract (Flexible Schedule)
We are seeking a Clinical Case Manager to provide mental health support services to patients within a growing behavioral health practice. This role offers flexibility in hours, multiple employment options, and the opportunity to earn additional income through a productivity-based compensation model.
This position may be structured as full-time, part-time, or contract, depending on the candidate’s availability and preferences. Candidates seeking to work more hours may elect a heavier productivity-driven reimbursement model.
Position Overview
The Clinical Case Manager will support patients by helping them navigate mental health care, reduce barriers to treatment, and access necessary services and resources. Services are provided through a combination of telehealth, phone, and in-person support.
Caseloads may be mixed age wise, so comfort working with different age groups is preferred.
Key Responsibilities
- Provide case management and care coordination services for mental health patients
- Conduct patient outreach, follow-ups, and check-ins via phone, telehealth, and in person
- Assist patients with treatment engagement, appointment coordination, and access to services
- Help patients navigate insurance, benefits, applications, and community resources
- Complete accurate documentation and billing within the EHR (TherapyNotes)
- Provide advocacy and support to reduce barriers to care
- Participate as a collaborative member of a multidisciplinary team
Schedule & Work Environment
- Flexible scheduling (hours may vary based on availability and productivity goals)
- Primarily telehealth-based, with some in-person services required as needed
- May include:
- Home visits
- Community-based support (transportation coordination, resource linkage, advocacy)
- Reliable transportation required for in-person services
Compensation
- Hourly pay, with additional compensation tied to productivity and billable services
- Opportunity to increase earnings based on motivation and caseload engagement
- Compensation structure discussed during the interview process
Qualifications
- Bachelor’s degree in Human Services, Social Work, Psychology, or a related field (preferred), minimum a high school diploma to apply.
- Experience in mental health or behavioral health settings strongly preferred
- Case management experience a plus
- Comfortable using technology and electronic health records (TherapyNotes experience preferred)
- Strong communication, organizational, and problem-solving skills
- Ability to work independently while collaborating with a team
- Comfortable working with both adolescents and adults
