Role Description
Holistic, Inc. is seeking a compassionate and dedicated Social Worker for a full-time or part-time hybrid role based in Charleston, WV or ST Albans, WV. The Social Worker will support patients through direct counseling and by connecting individuals and families to the resources they need for mental, emotional, and physical well-being. Responsibilities include conducting needs assessments, ongoing case management, developing individualized care plans, and facilitating access to community resources. Some remote work flexibility is available for this role.
Qualifications
- Experience in counseling, client advocacy, case management, and providing emotional support to individuals and families.
- Proficiency in conducting assessments, creating care plans, and connecting patients to community resources through referrals.
- Strong verbal and written communication skills, along with organizational and administrative abilities for managing multiple cases.
- Empathy, active listening skills, and an understanding of mental health and social care challenges faced by diverse populations.
- Master's degree in Social Work or equivalent, with licensure or eligibility for licensure as a Social Worker in West Virginia.
- Knowledge of local community resources and experience working in a healthcare or counseling setting is highly desirable.
