Oriol Health Care is looking for a Per Diem Social Worker to support both Holden Rehab (Holden, MA) and Oakdale Rehab (West Boylston, MA) on an as need basis (few times per year). The Social Worker will be under the general supervision of the Facility Administrator and/or Director of Nursing; the Social Worker is responsible for providing all coordination necessary with bringing residents into the facility and ensuring plans are established for emotional and social support.
Pay Range: $34.00 - $44.00/hr
Responsibilities:
- Coordinates and participates in family and resident activities to promote social interaction, reality orientation, and intellectual stimulation.
- Assesses resident and family situation and ascertains which members are involved in resident care planning.
- Counsels residents and family in dealing with feelings about death, dying, as well as other emotional, mental, environmental and physical concerns.
- Practices profession in accordance with all federal/state regulations.
- Documents emotional and social needs of residents and suggest course of action
- Refers residents to social, health, and community agencies while ensuring completeness of documentation of referrals
- Acts as a liaison with social, health, and community agencies
- Coordinates family and community resources to solve financial needs
- Organizes, plans, schedules, and implements room visits and in-room activities for residents unable to leave their rooms.
- Coordinates appropriate activities to meet resident social needs
- Works with Director of Nursing to complete MDS sheets
- Acts as member of various committees as resident advocate
- Adheres to corporate/departmental policies and procedures
- Operates in accordance with all safety regulations
Knowledge, Skills and Abilities Required:
- Minimum required of a 4-year college degree
- State Social Worker license required!
- 3 years of LSW experience
- Extensive understanding of all applicable federal/state regulations
- Maintains ongoing education to changes/trends in industry
- Excellent communication and interpersonal skills
- Ability to develop one-on-one relationships with resident/families/practitioners
- Valid driver's license
Physical and/or Visual Abilities Required:
- Position requires minimal physical requirements and visual abilities
