Outreach Coordinator 1 – Housing Navigation Services

Summary: We are seeking a detail-oriented and compassionate Outreach Coordinator to join our Housing Navigation team. This position serves as the first point of contact for members referred to our program and plays a critical role in ensuring timely outreach, insurance eligibility verification, and accurate documentation. The ideal candidate has excellent communication skills, strong computer proficiency, and enjoys working in a fast-paced, team-oriented environment while making a positive impact in the community.
Key Responsibilities
- Verify member insurance eligibility and benefits using payer portals and eligibility systems.
- Complete first-contact screenings to our members through both manual calls and an automated dialer system.
- Accurately document all member interactions and outreach attempts in company systems.
- Create, update, and maintain spreadsheets used to track referrals, outreach efforts, authorizations, and productivity.
- Upload and organize documentation into electronic records and databases.
- Monitor referrals to ensure timely outreach and follow-up in accordance with program requirements.
- Collaborate with Housing Navigators and medical providers.
- Maintain confidentiality and comply with HIPAA regulations.
- Meet productivity and quality standards while providing exceptional customer service.
- Perform additional administrative duties as assigned.
Qualifications
- High school diploma or equivalent required; Associate's degree preferred.
- Minimum of 1 year of phone customer service or call center experience.
- Experience working in a medical office, healthcare, insurance, or social services setting is highly preferred.
- Experience verifying insurance eligibility is a plus.
- Strong computer skills, including proficiency in Microsoft Excel, Word, Outlook, and Google Workspace preferred.
- Ability to create and maintain spreadsheets and accurately enter data.
- Excellent verbal and written communication skills.
- Professional telephone etiquette and active listening skills.
- Strong organizational skills with exceptional attention to detail.
- Ability to manage multiple priorities and work independently in a fast-paced environment.
- Bilingual (English/Spanish) is preferred.
