Primary Duties and Responsibilities
- The Patient Educator (RN) provides individualized nursing education to patients and families using an evidenced-based, multi-disciplinary approach.
- This role ensures the patient has the knowledge to care for his/her condition and is educated and empowered to participate in the plan of care.
- Serves as a liaison among clinical service staff, physicians, education departments, and patients.
- Provides health assessments, plans and reassesses as well as makes appropriate referrals for outside services/continued care.
- Participates in data collection and analysis of clinical outcomes.
- Assists with education of clinical professionals.
- Keeps current in best practices by attending conferences, educational programs and continuing education classes.
- Functions both in inpatient and outpatient environments.
- May develop education programs and facilitate educational classes.
- Performs other duties as assigned.
- As an Inpatient Diabetes Educator, utilizes clinical expertise to identify educational needs of patients who are newly diagnosed with diabetes.
- Demonstrates the ability to work independently in a self-directed manner to provide diabetes education and support.
- Performs learning needs assessments using the ADCES7TM framework.
- Initiates diabetes self-management.
- Educates patients and family about hyperglycemia and its management.
- Ensures close collaboration with Mary and Dick Allen Diabetes Center, hospital personnel, and discharging patient for outpatient diabetes self-management and training.
- Documents status of diabetes education and communicates diabetes-related patient needs to other members of the healthcare team.
- Provides nursing care in accordance with professional, regulatory standards and evidenced based practice.
- Maintains standards of nursing care as recommended by the Nursing Departments philosophy and objectives.
- Supports the hospital staff in the implementation of “management of inpatient hyperglycemia.”
Education and Experience Required:
2 or more years of clinical nursing experience.
License Required
Registered Nurse (RN) in the state of California - Current and in good standing.
Certifications Required

- Diabetes: At the end of two years the RN educator must become “ Certified Diabetes Care and Education Specialist”
