Primary Duties And Responsibilities
- The Rehab Admissions Liaison is responsible for proactively identifying clients who meet program criteria and managing the assessment admission process into the Fudge Family Acute Rehabilitation Center.
- Provides education on acute rehabilitation services and resources including participation expectations and hours of therapy per day with current and potential patients, case managers, physicians, and outside agencies.
- Assists patients with the process for admission to the rehabilitation hospital.
- Determines eligibility by verifying coverage information, ensuring that the patient meets Inpatient Rehabilitation Facility (IRF) criteria and securing physician acceptance/approval by providing detailed documentation in the pre-admission screen.
- Facilitates benefit eligibility and precertification including providing a plan of care and participating in external payer negotiations.
- Serves as the liaison case manager with the internal and external case managers.
- Maintains optimal unit census through coordination of admissions and discharges while staying informed and compliant with 60/40 CMS rule.
- Develops strategies to grow market share.
- Provides data management and outcome analysis.
- Performs other duties as assigned.
Education and Experience
- Bachelor’s Degree required.
- Previous experience with multiple payers, and insurances required.
- Two years’ clinical experience with experience in acute rehabilitation preferred.
- Experience with Medicate IRF admission criteria preferred.
- Bilingual or multilingual preferred.
License Required
- Current California licensure required, one of the following: Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), Speech-Language Pathologist (SLP), or Licensed Clinical Social Worker (LCSW). Valid CA Driver’s License required.
