About the Role:
As a Patient Navigator LPN, you will play a crucial role in ensuring that patients receive the best possible care. You will be responsible for guiding patients through the healthcare system, providing them with the support and resources they need to navigate complex medical procedures and treatments. Your primary goal will be to ensure that patients receive timely and effective care, and that their needs are met throughout the entire healthcare process. You will work closely with physicians, nurses, and other healthcare professionals to coordinate care and ensure that patients receive the highest quality of care possible.
Minimum Qualifications:
- Active LPN license in the state of employment.
- Minimum of 2 years of experience in a healthcare setting.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Excellent organizational and time management skills.
Preferred Qualifications:
- Experience working as a patient navigator or care coordinator.
- Experience working with electronic medical records (EMRs).
- Bilingual in English and Spanish.
- Experience working with underserved populations.
Responsibilities:
- Assist patients in navigating the healthcare system, including scheduling appointments, coordinating care, and providing support and resources as needed.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure that patients receive timely and effective care.
- Maintain accurate and up-to-date patient records, including medical histories, test results, and treatment plans.
- Educate patients and their families about their medical conditions, treatment options, and available resources.
- Provide emotional support to patients and their families throughout the healthcare process.
Skills:
As a Patient Navigator LPN, you will use your strong communication and interpersonal skills to build relationships with patients and their families, and to collaborate effectively with healthcare professionals.
