Social Worker - Respiratory and Rehabilitation Center in Coventry
Job Summary:
We are seeking a compassionate and motivated Social Worker to join our team at Respiratory and Rehabilitation Center in Coventry. As a Social Worker, you will provide emotional, social, and practical support to residents and their families, while working collaboratively with our interdisciplinary team to ensure the best possible resident experience.
Responsibilities:
Provide social services support to residents and families
Assist with admission, adjustment, transfer, and discharge planning
Support residents with emotional and psychosocial needs
Coordinate with nursing, therapy, administration, and outside agencies
Maintain accurate and timely documentation
Help resolve resident and family concerns in a professional and caring manner
Ensure compliance with all facility policies and applicable regulations
Qualifications:
Previous experience as a Social Worker in a healthcare or long-term care setting preferred
Strong communication and interpersonal skills
Compassionate, organized, and team-oriented
Ability to manage multiple tasks and work effectively with residents, families, and staff
Knowledge of discharge planning and resident rights preferred
