What You’ll Do
- Act as an advocate and emotional support system for residents and their families.
- Provide guidance on resident rights, care planning, and transitions of care.
- Coordinate services and resources to enhance resident wellbeing — from counseling to arranging personal items or support services.
- Serve as a key member of our interdisciplinary team, working closely with staff and family to address individual needs.
- Support residents through grief, adjustment, and life changes with compassion and care.
- Document social services needs and ensure individualized care plans reflect evolving goals.
- A licensed Social Worker in good standing, or a degree in social work or related human services field (sociology, psychology, gerontology, etc.).
- At least one year of supervised social work experience in a healthcare setting.
- Someone who is patient, empathetic, a great communicator, and thrives in a fast-paced, resident-first environment.
- Flexibility and autonomy in a leadership role.
