Job Summary
LCSW (Social Worker) - Sacramento/Represa, CA
Full-time, Contract (27 weeks, with the opportunity to extend)
The hourly rate will depend on whether you are working as a W2 or 1099
Schedule: 7 am - 3 pm Mon, Tues and Wed
Hourly Wage: Minimum 30 hours per week, paid weekly
The Opportunity
This is a long-term contract that will renew if all goes well. You will work in a restricted area conducting 1-on-1 assessments for patients. Many will have mental health, behavioral or suicide ideation. The work will be challenging and rewarding.
Responsibilities
Provide clinical social work services to clients, including assessments and treatment planning.
Conduct psychosocial assessments and develop individualized care plans.
Maintain accurate documentation and ensure compliance with regulatory standards.
Coordinate referrals and collaborate with multidisciplinary teams for holistic care.
Monitor client progress against targets and update plans accordingly.
Provide crisis intervention and risk assessment as needed.
Deliver evidence-based social work practices and pursue ongoing professional development.
Adhere to a performance-driven culture; use pipelines to forecast outcomes.
Engage in community outreach to expand services and pipeline opportunities.
Educate clients and families about treatment options, resources, and benefits.
Assist clients with navigating community resources and support services.
Participate in weekly team meetings, sharing updates and best practices.
Experience/Qualifications
Licensed Clinical Social Worker (LCSW) in California with current license.
Minimum of 1 years of clinical social work experience with assessments and treatment planning.
Proficient in psychosocial assessments and individualized care plan development.
Strong documentation skills ensuring regulatory compliance and privacy standards.
Crisis intervention and risk assessment capabilities with safety planning.
Evidence-based practice knowledge and commitment to ongoing professional development.
Experience coordinating referrals and collaborating with multidisciplinary teams.
Ability to use performance metrics and pipelines to forecast outcomes and drive results.
Community outreach experience to expand services and pipeline opportunities.
Must be able to undgo and pass a background check and drug screen
Must have reliable transportation to work
