Hinds LIfeCare, one of the most well-known and well-respected nonprofit agencies in California’s Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a Full Time, Medical Social Worker / LCSW to join our team.
Under operational direction of the performs the role of social worker as a member of the interdisciplinary team. The Hospice Social Worker completes psychosocial assessments and provides ongoing psychosocial support to patients and families.

Qualifications:
- Compensation $40 - $41 hourly

Qualifications include but are not limited to:
- Master's degree (MSW) from a School of Social Work accredited by the Council on Social Work Education
- Minimum one (1) year of Medical Social Work experience required; Palliative, Hospice and or Home Health Care preferred
- Electronic medical record experience preferred
- Ability to learn and assimilate the EHR Computer Program/System
- Demonstrated ability to work with health team personnel Bilingual (English/Spanish) a plus but not required
- Must have a valid drivers license and a willingness to use personal vehicle in the course of employment
- Must show proof of automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36 month period)
- Ability to meet the physical demands of the job (ie, Push, Pull, Lift/Carry 10 lbs)
