Hospice Care Consultant (37884)

Hinds Lifecare, one of the most well-known and well-respected nonprofit agencies in California’s Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a  Hospice Care Consultant to join our team.

The Hospice Care Consultant serves as the liaison between Hinds Lifecare and Skilled Nursing Facilities within Fresno, Madera, and Merced counties and maintains regular and ongoing contact with physicians offices.

• Determine potential referral sources, then contact and educate about Hinds Lifecare services.
• Coordinate and conduct presentations on end-of-life care to healthcare professionals and the general community. Coordinate general community education and outreach activities (e.g. health fairs, United Way campaign, etc.) to promote Hinds Hospice programs and services.
• Promote public relations in all activities of the organization (e.g. fundraising events, on-going promotions, thrift stores, new programs).
• Develop and foster relationships with existing referring and non-referring physicians and other healthcare providers providing them information on Hinds Lifecare end-of-life care services.

• Compensation $80K to $90K and eligible for quarterly incentive bonus

Qualifications include but are not limited to:

• Bachelors Degree with a minimum of one year experience in Hospice Care Coordination is required.
• Experience in marketing/sales highly desirable.
• Excellent communication skills.
• Ability to interact with the public, vendors and other employees in a professional manner required.
• Must have a valid CA drivers license and a willingness to use personal vehicle in the course of employment.
• Must show proof of automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36 month period).
• Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 10 lbs.)
