Role Description
This is a full-time on-site role for a Social Worker located in Jamaica, NY. The Social Worker will be responsible for assessing the social and emotional needs of residents, developing individualized care plans, and providing counseling and support to residents and their families. Day-to-day tasks include coordinating with healthcare providers, arranging community resources, and documenting resident progress. The Social Worker will also advocate for the rights and well-being of residents.
Qualifications
- Assessment, Care Planning, and Counseling skills
- Knowledge of community resources and coordination with healthcare providers
- Advocacy and Documentation skills
- Strong interpersonal and communication skills
- Ability to work collaboratively in a healthcare setting
- Relevant social work licensure and certification
- Bachelor's or Master's degree in Social Work or related field
- Experience in a rehabilitation or extended care setting is a plus
