Job Overview
Hillsdale County Senior Services Center is seeking a dedicated and highly skilled Assessment Specialist to join our team. The ideal candidate will possess a comprehensive understanding of phone and home assessments and will adhere to HCSSC organizational standards set by procedures and policies. This role involves conducting thorough client assessments, developing individualized meal plans with the Nutrition Department. The Assessment Specialist will play a vital role in ensuring accurate data collection, documentation, and compliance with state and federal regulations, HCSSC and funding source policies. The position offers an engaging opportunity to make a meaningful impact on patients' lives through expert evaluation and compassionate support.

Responsibilities

  • Conduct detailed patient assessments.
  • Collaborate with nutrition staff and to ensure nutrition needs are met.
  • Maintain compliance with HIPAA regulations by safeguarding patient information. during all stages of assessment and documentation.
  • Ensure accurate detailed information.
  • Assist clients through contacts with appropriate staff, physicians, and other community organizations.

Requirements

  • Two-year certificate from college or 1-3 years related training or experience or a combination of both.
  • Valid driver's license
  • Positive background and physical results.
  • Must have the ability to master technology
  • Excellent communication skills combined with the ability to work collaboratively within multidisciplinary teams committed to the community we serve.

Pay: $13.63 - $15.00 per hour
