Job Summary:

A physical therapist provides professional services in restorative, functional, and preventative therapeutic programs working directly with the patient, physicians, and a multidisciplinary team to optimize patient outcomes. This includes evaluation of all assigned patients, the development of goal-oriented treatment plans, administration and implementation of the treatment program, discharge planning, and management of the patient’s physical therapy care.

The Therapist is responsible for providing supervision to assistants, students and aides within standards set forth in the Illinois State Physical Therapy Practice Act and accrediting body agencies.

Essential Duties and Responsibilities:

  • Evaluate patients through skilled examination, testing, and clinical assessment to determine functional limitations and rehabilitation needs.
  • Develop individualized treatment plans with measurable goals based on physician referral, patient needs, and evidence-based practice.
  • Provide physical therapy interventions per an established plan of care with continuous evaluation of patient tolerance, progress, and skilled medical necessity.
  • Educate patients, families, and caregivers regarding diagnosis, treatment plans, home exercise programs, safety precautions, and wellness strategies.
  • Monitor patient progress and modify treatment plans as appropriate to maximize outcomes and functional independence.
  • Ensure patient safety during all therapy activities and follow infection control facility safety policies.
  • Accurately document evaluations, daily treatment notes, progress reports, discharge summaries, and all required clinical documentation in a timely manner per department policies.
  • Collaborate effectively with physicians, nursing staff, case managers, therapists, and other interdisciplinary team members to coordinate patient care.
  • Maintain compliance with all regulatory, licensing, accreditation, and payer requirements, including Medicare and insurance guidelines.
  • Participate in discharge planning and make recommendations regarding equipment, assistive devices, community resources, and follow-up services.
  • Supervise and delegate appropriate tasks to physical therapist assistants, aides, and students in accordance with state practice acts and organizational policies.
  • Maintain patient confidentiality and uphold professional standards in accordance with HIPAA, State of Illinois Practice Act and Code of Ethics, and the American Physical Therapy Association Practice Act and Code of Ethics.
  • Participate in departmental meetings, quality improvement initiatives, and staff development activities.
  • Maintain a safe, clean, and organized treatment environment and ensure proper care and use of therapy equipment.
  • Promote exceptional customer service and patient satisfaction through compassionate, professional care.
  • Complete other duties assigned to meet the needs of the clinic and/or organization

Professional Requirements:

  • Completes and maintains all required education and/or training requirements.
  • Complies with HIPAA regulations and always maintains patient confidentiality.
  • Follows and maintains all lawful safety, health requirements, and regulations, and follow protocols for required protective equipment (PPE).
  • Promptly reports safety issues, job-related injury, or illness, near misses, etc. in SafetyZone.
  • Follows Hillsboro Health Standards of Behavior.
Requirements:

Education and/or Experience:

  • Minimum of a Bachelor of Science Degree from an APTA accredited curriculum with a degree in Physical Therapy
  • One year of physical therapy experience preferred

Certificates and Licenses:

  • Eligible for Illinois State Licensure in physical therapy and meet all requirements necessary for renewal
  • Valid driver’s license
  • Basic Life Support (BLS) required or willing to obtain within 30 days of hire.

Physical Demands:

Physical demands of the position, including vision, hearing, repetitive motion, and environment are required. A separate document will be provided upon hire, during annual evaluation process, and upon request. CORPORATE COMPLIANCE

Receives training and/or attends necessary meetings to meet the criteria as outlined in Hillsboro Health’s Corporate Compliance Plan and Code of Conduct. Understands the responsibilities related to compliance and knows whether to contact the Corporate Compliance Officer should there be any instance of question or concern regarding fraud and/or abuse.

Salary : $42 - $52
