Get to Know Your Team:
- Utica Park Clinic, founded in 1982, is a multi-specialty medical group with more than 300 employed physicians and advanced practice providers representing more than 30 specialties across 82 sites in Oklahoma, serving 21 communities.
- The Clinical Navigator RN coordinates and facilitates patient care across the continuum of care for designated patient populations.
- The position serves as a clinical resource, patient advocate, and liaison among patients, families, providers, and interdisciplinary teams to promote high-quality outcomes and a positive patient experience.
- This role supports care coordination, patient education, clinical program initiatives, and evidence-based practice standards.
- The Clinical Navigator RN contributes to improved clinical outcomes through effective communication, collaboration, and patient-centered care management.
Job Requirements:
- Graduate of an accredited school of nursing.
- Current Registered Nurse (RN) license.
- BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Experience in cardiology, vascular services, critical care, cardiac surgery, or emergency services.
- Public speaking or community education experience.
- Knowledge of quality improvement methodologies.
- Skill in program coordination and patient navigation activities.
