Hillcrest Health and Living is currently seeking an Clinical Care Coordinator to join our Hospice Iowa team!
This position will be responsible to assist the Director of Operations with the coordination and leadership to all of the direct care team members of the hospice team.

- Assist with coverage of visits and/or admissions as needed.
- Assists in monthly facility chart audits and ALOS audits.
- Participates in service line meetings as required.
- Supervises clinical team members completing supervisory visits as needed.
- Assist Director of Operations with the oversight of payroll each pay period.
- Answers calls from the team, patients/families and providers. Take orders and process as necessary.

- At least two years of RN experience in a hospice setting.
- At least two years of leadership experience in a hospice setting preferred.
- Must be licensed in the state of Iowa or compact state as an Registered Nurse.
- Must have a valid driver’s license as well as reliable means of transportation and carry current automobile insurance.
