Job Summary and Qualifications

This position will be a combination of Case Management and Infection Prevention. Travel required for training, to be discussed upon interview.

Schedule: Monday-Friday, 8hr shifts, 36hrs a week

The Registered Nurse (RN) CM is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team. The RN CM will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

What qualifications you will need:

• Required Experience: 2 yrs of case management or 3 yrs of clinical nursing experience
• Preferred Experience: InterQual and or Infection Prevention experience
• Required Education: Bachelor of Science in Nursing or enroll in a BSN or advanced degree program and obtain a BSN within 5 years of hire date into RN position. Grandfathering will be accepted between departments and Mission Health entities. Exemptions, or extensions, may be approved by the facility CNO and reported out to the CNO Council members. Each facility will ensure tracking of exemptions should this information be needed. RNs with 10 years or more of experience are exempt.
• Preferred Education: Master's Degree in Nursing
• Required Certification: BLS
