What are the responsibilities and job description for the Social Services Director position at Highland Pines Center?
Highland Pines Center is hiring for an experienced Social Services Director (Social Worker) to join our team!
Our comprehensive programs meet the changing needs of both our short-term patients recovering from surgery or an acute illness, as well as the needs of our long-term residents.
The ideal candidate must possess:
- Working knowledge of care plans and discharge planning services
- The ability to assist potential and existing residents and patients with the Medicare and Medicaid application process.
- Minimum 2-5 years Long-Term Care experience strongly preferred
- Minimum 2-5 years of employment experience in a social services industry or department
- BSW and/or MSW required
