Summary

Provides mental health services as defined by the California State Department of Mental Health. Responsibilities include case management for residents, including planning, coordination of services, monitoring progress, and collaboration with other agencies. Additional duties may be assigned as needed.

Essential Duties & Responsibilities

Direct Service and Case Management

  • Provide culturally competent services to all residents without regard to race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, disability, or payment status
  • Deliver counseling, training, and assistance with daily living skills and psychiatric symptom management
  • Support residents in accessing medical, educational, vocational, rehabilitative, and social services based on individual needs and goals
  • Develop and implement cost-effective service plans in collaboration with residents, families, staff, and service providers
  • Implement preventative strategies and crisis interventions when necessary
  • Complete Annual Assessments in collaboration with residents, families, and providers
  • Inform residents of their rights, responsibilities, and grievance procedures, ensuring freedom of choice
  • Transport residents to appointments and community outings; maintain vehicle inspection logs as required
  • Provide education to residents, families, and the community on health care prevention, early detection, and treatment

Teamwork and Collaboration

  • Maintain professional and cooperative working relationships
  • Contribute feedback to management to improve program quality and effectiveness
  • Participate in monitoring program outcomes and resident satisfaction

Reporting and Documentation

  • Comply with mandated reporting requirements for suspected abuse of dependent adults and elders
  • Monitor and document resident progress toward service plan goals
  • Complete all required documentation (e.g., assessments, service plans, progress notes) within established timelines
  • Maintain accurate and professional records in resident files and communication logs
  • Meet documentation standards, billing expectations, and participate in quality improvement activities (QIC) as required

Activities (As Assigned)

  • Plan and lead group and recovery-oriented activities
  • Encourage resident participation in planning, preparing, and evaluating activities
  • Follow established monthly activity schedules

Qualifications

Education and/or Experience

  • Bachelor’s degree in Social Work or a related field, or
  • Minimum of 2 years of experience working in mental health

Knowledge and Skills

  • Knowledge of resident care and supervision requirements
  • Understanding of HIPAA, OSHA, and applicable state/local regulations (including Title 22 and Community Care Licensing)
  • Strong verbal and written communication skills in English
  • Ability to effectively present information and respond to questions
  • Basic math skills related to job tasks (percentages, proportions, etc.)
  • Strong problem-solving and organizational skills
  • Ability to manage multiple priorities and meet deadlines
  • Proficiency with computers (email, documentation systems, word processing)

Additional Requirements

  • Must be at least 18 years of age (21 if driving is required)
  • Valid driver’s license and insurable under company policy (if applicable)
  • DOJ and FBI clearance required prior to employment
  • TB clearance within the last 6 months and successful health screening
  • Valid First Aid certification
  • Completion of all required annual trainings and compliance requirements
  • Attendance at required meetings and trainings

Physical Demands

  • Ability to sit, stand, and walk for extended periods
  • Frequent reaching, bending, stooping, and occasional lifting up to 50 lbs
  • Must be physically and mentally able to perform duties in accordance with regulatory standards

Work Environment

  • Typically low to moderate noise levels
  • Possible exposure to cleaning chemicals
  • May encounter unpredictable resident behavior, including verbal or physical interactions
