**Position:** Certified Diabetes Educator (CDE)
**Location:** Outpatient Maternal-Fetal Medicine Office, Phoenix, AZ
**Employment Type:** Full-Time
**Overview:**
We are seeking a dedicated and experienced Certified Diabetes Educator (CDE) to join our Maternal-Fetal Medicine office. The CDE will play a critical role in managing diabetes during pregnancy, ensuring the best possible outcomes for both mothers and their babies. The ideal candidate will have a strong background in diabetes education and management, particularly in a maternal-fetal medicine setting.
**Key Responsibilities:**
- **Diabetes Management:** Provide comprehensive diabetes management for pregnant patients, including diet education, blood sugar monitoring, and medication management.
- **Diet Education:** Educate patients on dietary changes and meal planning to manage blood glucose levels effectively.
- **Insulin Management:** Prescribe and adjust insulin doses as necessary, manage insulin pumps, and continuous glucose monitoring systems.
- **Patient Education:** Conduct individual and group education sessions on diabetes self-management, emphasizing the importance of glucose control during pregnancy.
- **Monitoring and Adjustments:** Monitor patients' blood glucose levels, make adjustments to treatment plans as needed, and manage hypoglycemia.
- **Collaboration:** Work closely with physician , and other healthcare providers to develop and implement individualized care plans.
- **Documentation:** Maintain accurate and up-to-date patient records, including treatment plans, progress notes, and any changes in therapy.
- **Support and Counseling:** Provide ongoing support and counseling to patients to help them cope with the challenges of managing diabetes during pregnancy.
**Qualifications:**
- **Certification:** Current Certified Diabetes Educator (CDE) certification required.
- **Experience:** Minimum of 3 years of experience in diabetes education and management, with a focus on maternal-fetal medicine preferred.
- **Education:** Bachelor’s degree in Nursing, Dietetics, or a related healthcare field. Master’s degree preferred.
- **Skills:** Proficient in managing insulin pumps, continuous glucose monitoring systems, and prescribing and adjusting insulin doses.
- **Communication:** Excellent communication and interpersonal skills, with the ability to educate and motivate patients.
- **Licensure:** Active RN or RD licensure in the state of Arizona.
