Now Hiring: Registered Nurse (RN) or Licensed Practical Nurse (LPN)
Schedule: Every other weekend 4-10 hr shifts or 3-12 hr shift
Status: Full-Time (daytime)
About the Role
High Point Residence – Centralia is seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Memory Care team. This role is essential in delivering high-quality, resident-centered nursing care in accordance with individualized assessments and care plans, under the direction of the Wellness Director.
The ideal candidate is clinically strong, team-oriented, and passionate about serving residents living with memory loss.
Position Responsibilities
Provide routine daily nursing care and clinical services to residents
Perform vital sign checks and comprehensive health assessments
Collaborate with CNAs and care staff to monitor resident conditions and needs
Administer medications and ensure accurate, timely documentation
Provide clinical supervision, guidance, and leadership to care staff
Support a safe, positive, and resident-focused environment
Perform additional duties as assigned by the Executive Director
Qualifications
Current Illinois RN or LPN license in good standing
Recent experience working in a Memory Care or senior living environment
Strong clinical judgment, assessment, and supervisory skills
Experience with medication administration and electronic documentation systems
CPR and Food handlers preferred
pay negotiable based on LTC, or Dementia care experience
Physical & Certification Requirements
Ability to lift up to 25 pounds and assist with resident care as needed
Valid CPR certification (recently expired certifications accepted; recertification available)
