About Us

Our Transitional Support Services Programs (TSS) offer services that bridges the gap between detox and step-down services to residential after-care.

High Point & Affiliated Organizations is a health and human service agency whose mission is to treat and prevent substance use disorders and mental illness. High Point has programs located throughout Southeastern Massachusetts offering a full continuum of care for substance use and mental health treatment, including inpatient, outpatient, residential, and community-based services. Programs and services also assist survivors of abuse, violence, and families experiencing homelessness. High Point believes that everyone has inherent goodness, worth, and dignity. Our goal is to help individuals and families achieve personal change and improve their quality of life.

Medication Specialist Requirements

  • Must present as courteous and professional at all times;
  • Demonstrated flexibility and ability to perform multiple tasks;
  • Must have excellent communication skills, including the ability to organize ideas in logical and clear fashion;
  • Knowledge of basic computer skills and software programs including the ability to learn, use and train HPAO Electronic Health Record (EHR);
  • Must have the ability to work with others in cooperative and collaborative manner;
  • An understanding of the relationship between substance use and mental health issues and/or self-help programs;
  • Understand and maintain therapeutic boundaries;
  • Reinforce rules in a respectful manner;
  • Comply with Federal Regulations, 42 CFR Part 2, Confidentiality of Substance Use Disorder and HIPAA Regulations;
  • TB/Flu and Covid19 Assessment;
  • Certification in the HPAO approved de-escalation program;
  • CPR Certification;
  • Successful completion of all trainings upon hire and annually as required.

Case Manager Duties & Responsibilities

  • Complete medication reconciliation and verify medications upon admission.
  • Accurately enter all medication into Electronic Health Record (EHR).
  • Ensure all Patients have at least a 30-day supply of current medication.
  • Orient new Patient to program’s medication policies and procedures.
  • Maintain an accurate record of any emergency medications which Patients have possession of.
  • Inform all Patients about Open Access when in need of medication and / or a new psychiatrist.
  • Set up new Patients with pharmacy, ensuring all paperwork is complete and sent.
  • Train newly hired staff in medication policies and procedures, including monitoring, refusals, incident reporting, entering / discharging medication in EHR, updating Methadone take home sheets, and how to scan forms into EHR.
  • Update pharmacy with any medication changes and insurance updates.
  • Request refills when medications are at 7 days or less.
  • Ensure ordered medications have been picked up from pharmacy.
  • Ensure medication are accurate and contact the pharmacy if there are discrepancies.
  • Update the Patient prescription binder to ensure all are current with medication, directions and prescriber.
  • Review missed medication log and meet with Patients regarding refusals / missed medication.
  • Communicate medication concerns, refusals, etc. to treatment team.
  • Document and dispose of medication according to policy.
  • Monitor medication during self-administration of medication at designated times and document in EHR with Patient signature.
  • Reconcile medication for Patient discharges.
  • Ensure the medication refrigerator is in working condition and monitor refrigerator temperatures.
  • Ensure medication area house supplies and program first aid kits are stocked and remove any expired supplies.
  • Maintaining the emergency contingency report daily in the event the EHR is unavailable for documentation.
  • Weekly cleaning of the medication storage areas.
  • Complete incident / environmental reports as required.
  • Complete and send medication errors / incidents to BSAS and insurance when required.
  • Arrange naloxone training for all staff yearly and as needed.
  • Provide medication education to all staff yearly and as needed.
  • All other duties as assigned.

Case Manager Qualifications

  • High School Diploma or equivalent GED preferred.
  • One-year experience in substance use treatment field (preferred but not required).
  • Prior experience with medication management preferred.
