This isnt traditional case managementthere are no long-term caseloads. Instead, youll provide in-the-moment support to help de-escalate crisis and help individuals find stability. Serve as a calm, steady presence during high-stress situations. Use trauma-informed de-escalation techniques to help individuals regain a sense of safety, control, and stability. Work closely with a multidisciplinary crisis response team, law enforcement, emergency services, and community partners to ensure coordinated, person-centered care. Provide short-term, impactful support designed to stabilize individuals in crisis and reduce unnecessary hospitalizations and emergency room visits. Develop safety plans, identify coping strategies, and connect individuals & families to community resources and ongoing supports once the immediate crisis has passed. Maintain clear, timely documentation of services. We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college.
